Mail orders are cheaper at $15 per copy with no extra fee. You need the official Birth Certificate Request Form from OSDH. Fill it out, include a copy of your photo ID, and mail it with a check or money order to: Vital Records Service, PO Box 248964, Oklahoma City, OK 73124-8964. Plan on about four weeks for processing.
Will Call pickup is available in Oklahoma City, Tulsa, and McAlester. Logan County residents will find the Oklahoma City office most convenient since Guthrie is only about 30 miles north of the capital. Pickup hours are 12:00 p.m. to 4:45 p.m., Monday through Friday. You must order ahead. Walk-in orders are not accepted anymore.

Logan County Birth Record Eligibility
Oklahoma law restricts who can get a certified birth certificate. Under Title 63, Section 1-323, access is limited to the person named on the record, a parent listed on the certificate, a legal guardian, or an attorney with authorization.
