Fill it out, include a copy of your photo ID, and send it with a check or money order to: Vital Records Service, PO Box 248964, Oklahoma City, OK 73124-8964. Plan on about four weeks. Do not send cash.

Will Call pickup is available in Oklahoma City, Tulsa, and McAlester. Osage County residents in the eastern part of the county may find Tulsa closest. Those in the western part may prefer Oklahoma City. Pickup hours run from 12:00 p.m. to 4:45 p.m., Monday through Friday. You must place your order in advance.

Osage County Birth Record Eligibility

Oklahoma law limits who can get a certified birth certificate. Under Title 63, Section 1-323, access is limited to the person on the record, a listed parent, a legal guardian, or an authorized attorney. Extended family such as spouses, grandparents, and adult children can request copies with proof of relationship.

Every request must include a clear copy of valid photo ID. The state accepts driver's licenses, passports, military IDs, and tribal photo IDs.
