Who Can Request Washita County Birth Records

Oklahoma keeps birth records confidential. Title 63, Section 1-323 says only specific people can get a certified copy. The person named on the record can request their own if they are of legal age. Parents on the certificate qualify. Legal guardians need court papers. Attorneys need signed authorization from the subject.

Family members like spouses, grandparents, and adult children can also request a copy. They need the subject's written permission and proof of the family link. A clear photocopy of government photo ID goes with every request. Do not send your original. Acceptable ID types include a driver's license, passport, military ID, and tribal ID with signature. Records 125 years or older are open to anyone, but you still must apply and pay.

Birth Certificate Filing for Washita County

The law in Title 63, Section 1-311 says the doctor or midwife at a birth must file a certificate with OSDH within five days. The form includes the child's name, date and place of birth, sex, and both parents' names. The mother's maiden name is also required.
